Student Forms

Course changes cannot be approved by instructors. All students MUST submit the appropriate forms below.
Due to fire codes, additional seats cannot be added to courses- NO EXCEPTIONS.

Registration Override Request Form 
Not to be used for Campus Override or Closed Class Override requests. 

Campus Restriction Override Request
For Campus Restriction overrides only. Campus Overrides are NOT guaranteed. You must provide justification for each request. Students are required to attend face-to-face on the designated campus.

Chair Override Request
To request class changes only after the 7th day of class.  Course changes will not be approved for an instructor change.  

Course Evaluation and Substitution Request
Use this form if you believe you have taken a course that should satisfy a computer science requirement for your degree. Complete and submit this form only once for each course evaluation request.

Closed Class Override Request
Students MUST utilize the waitlist feature in the registration system. Closed Class Overrides cannot be submitted at this time.

Closed class overrides are not standard practice. For undergraduate level courses, the department utilizes the wait-list function in the course registration system. Only in extremely extenuating circumstances will a closed class override be considered. 
Due to fire codes, additional desks/seats cannot be added to rooms.

Grade Appeal Form
To be submitted only after students have met with their instructors to review the assigned grade. 

Independent Study Course Proposal/Approval Forms
To allow sufficient time to complete the approval process and for timely student registration, the application form must be submitted no later than one week before the first day of class.
